K4 Form

This form is to be completed by the GP Specialty Training Co-ordinator (or nominated deputy) and GP Registrar. The information is collated to ensure that any registrar appointed to a GP Specialty Training Programme receives the correct salary and is placed on the correct pay scale. It is therefore the trainee's responsibility to complete the K4 and provide a copy of a current payslip, otherwise no payment will be made by SBS.

Registrar Mileage form for General Practice 

These forms are provided via the Practice not through the specialty programme. Registrars should be encouraged to claim monthly rather than at the end of the rotation.
