Patient satisfaction questionnaire


This is a fairly straightforward process, but there are a few catches:

  1. You need 40 completed forms, from consecutive patients. To do this you need to collect 50, as some patients do not successfully complete them.
     
  2. It takes longer than you think, so do not leave until the last minute.
     
  3. You should not personally handle the completed forms. They need to be sent straight from surgery in a sealed envelope to the VTS office.
     
  4. Someone in reception should be given some basic instructions on what to say to the patients.
     
  5. They should also check the forms and make sure they do not send in incomplete forms (usually a patient will have missed a single line of answers. Unfortunately this invalidates the whole form).
     
  6. Some trendy surgeries have a check in screen. Patients using these new fangled systems may avoid the receptionist and thus never be asked to complete a form.
     
  7. It is a real hassle having to repeat the process, so try and get it right first time.
